Staff-on-Call End-of-Shift Form

  • At the end of every on-call shift, the staff person should submit this form. ONLY fill in areas that require attention.
  • Anything that should remain confidential (e.g. personnel issues) should be emailed directly to your supervisor.
  • This form does NOT replace the Incident Report Form. Please see the guidelines for submitting an incident report

Incident Report Form 

  • Complete this to document and inform the library any time there is:
    • need of emergency responders
    • an issue with the building (e.g. water leak)
    • an issue with a patron
  • When in doubt, fill it out!

Report PATRON Card-Swipe Access Issues

  • Use this form any time a patron cannot swipe into Gumberg Library exterior doors on the 4th floor, the 3rd floor Wellness Center (Brottier Commons) entrance, or the COM library (limited to MBS and COM students, faculty, and staff only)
  • Refer employees who work in the building and need interior door access to Cindy Fischer (Assistant to the Dean of Libraries)

Group EVENT Stats

Guidelines for Recording Group Events

When should you use this form?

  • To record statistics after a group event has taken place?
  • Members of the library staff planning an event should follow the policies outlined on the Gumberg Library Toolkit.

Who should use this form?

If you were the primary contact for an event that was hosted in the library, after the event has ended you should complete this form even if it was led by another campus unit. Some examples include:

  • FAST Lab
  • Dissertation defenses in the Silverman Center
  • Classes hosted in the Curriculum Center

If you teach a class, organize a workshop, or hold a cultural event, after the event has ended you should complete this form (unless the event was only for library staff). See the ACRL Definition below.

Some examples include:

  • Information literacy / library instruction in classes
  • Therapy Dogs
  • Book Discussion
  • Art exhibits
  • EndNote workshops
  • Workshops open to faculty and graduate students 

When should you not use this form?

  • Events for library staff only
  • Events organized by other campus units but not held in the library and not taught by library staff

When should you complete this form?

  • For a session held in the library that is not planned or organized by the library, complete the first two columns
  • For a session planned or organized by the library, complete all three columns

  • This form goes to the Access Services Manager so they can collate, track, and anticipate need for supplies used in our office.
  • This is internal to ASD only.

HOME SAFE SERVICES
This program offers reimbursement for Uber/Lyft/Cab fare when an employee decides to call for a safe ride home when they find themselves too impaired to drive. The employee must obtain a receipt and mail to Lytle EAP Partners at 200 Cedar Ridge Drive, Suite 208, Pittsburgh, PA 15205. The receipt must contain “Duquesne University”, your name, address and telephone number. Reimbursement, limited to three times per year, up to $50 will be mailed to your home. EAP publicity materials are sent along with the reimbursement check.
