LibQUAL+ was originally developed by Texas A&M and is sponsored by the Association of Research Libraries. More than 1,000 institutions throughout the world have used LibQUAL+ to assess their libraries’ quality and performance.

Informed Consent Agreement
Procedure: This online survey should take approximately 10 minutes to complete.
Risks: The potential risks of participating in this study are minimal. However, some individuals may feel uncomfortable sharing their feelings about library service. If you become uncomfortable, you may discontinue your participation at any time by exiting the survey.
Compensation and Benefits: There is no direct individual compensation or benefits to participation in the survey. However, your participation will benefit the Duquesne community by helping us better understand your information needs.
Confidentiality: Your responses will be anonymous and confidential. Survey data is stored on servers kept in a biometrically secured, climate-controlled data center and is protected behind a firewall from the Internet. Due to the anonymous nature of the survey itself, SSL encryption is not used on the survey form. Responses will be stored, analyzed, and reported in aggregate.
Withdrawal without penalty: Participation in the survey is completely voluntary, and you may discontinue your participation at any time by exiting the survey. Refusal to participate or discontinuing participation will involve no penalty or loss of benefits.
Public use of data: Information from the survey will be presented to the campus community through presentations and the Gumberg Library homepage. Information may be used in conference presentations or publications in academic journals. Aggregate responses will be shared with other LibQUAL+ Lite participants in order to compare Gumberg Library’s performance with other academic libraries. Your identity will not be disclosed at any time.
Incentives: Participation in survey incentives is completely voluntary. Your contact information will be collected only if you would like to enter the drawing for survey prizes. Your contact information will not be connected to your responses.
Eligibility: Participation in this survey is limited to Duquesne University students, faculty, and staff over the age of 18.
